- Alcohol: 5–10 days of physician monitoring depending on severity, with peak seizure risk at 24–72 hours and continued monitoring through day 10 for heavy-use presentations.
- Benzodiazepines : 10–14 days of supervised taper, depending on the specific benzo, dosage, and duration of use. Long-acting agents (Klonopin, Valium) require the longest taper protocols.
- Fentanyl: 7–10 days for the acute medical phase. Onset at 8–24 hours, peak intensity at 36–72 hours, with extended management needed due to fentanyl's lipophilicity.
